Flexible Payment Options including Medicare
Louisville Sleep Health is in-network with Medicare. For those with other health plans, the services we provide are recognized and may be reimbursed through out-of-network benefits. Please contact your insurance carrier to inquire about your out-of-network benefits. We will be happy to provide any requested information required (commonly used CPT codes) upon request. We will also provide you with a “Superbill” at time of service and provide assistance in the filing of your claim if requested. Payment or payment arrangements for services must be made day of or prior to date of service.
Other accepted methods of payment include HSA, HRA, credit card, check, or cash.
